This is a financial power of attorney. You can grant your attorney-in-fact the power to deal with a number of matters for you, including:

When does the power of attorney become effective?

You can decide whether to make the power of attorney come into effect immediately after you sign it, or only if you are determined to be mentally incapable of handling your own affairs.
